Why we collect your personal data and what we do with it:
When you supply your personal details to Standen Physiotherapy, it is stored and processed for the following reasons:
We need to collect personal information about your health in order to provide you with the best possible treatment. Your requesting treatment, along with our agreement to provide that care, constitutes a contract. You have a right to refuse to provide this information; however, this would prevent the treatment process being provided.
We have a "Legitimate Interest" in collecting this information. Without it, we are unable to provide our an effective and safe service.
We may need to contact you in order to confirm your appointment, or to update you on matters related to your medical care. This again constitutes "Legitimate Interest" but in your own interest.
Provided we have your consent, we may occasionally send you general health information in the form of articles, advice or newsletters. You may withdraw this consent at any time - just let us know by any convenient method.
We have a legal obligation to retain your records for 7 years after you most recent appointment. After this period, you can ask us to delete your records if you wish. We will retain your records otherwise, in order to provide you with the best possible care should you need to see us at some point in the future.You have the right to see what personal data of yours we hold and you can also ask us to correct any factual errors.
Your records are stored:
On paper in a locked filing cabinet using a key safe. This is always kept in a locked room at all times.
We currently do not use electronic records; however, the work computer is two tiered password protected, backed up regularly and kept in a locked room out of working hours.
We never share your data with anyone who does not need access without your consent. Only your physiotherapist will have access to you data. In addition to any circumstances described above, we may share any data we collect if we are required to share data with the legal authorities to fulfil our obligations in accordance with the laws of England and Wales.
